For families who need documentation to kickstart the 504 process.

 

Includes:

• Full mental health evaluation for Depression, Anxiety, and/or ADHD, including:

  • Individual & Family Questionnaires/Inventories based on DSM-5 criteria

  • Clinical Diagnostic Interview (60-minute virtual session; parent included if needed)

• Letter from a licensed therapist indicating any applicable diagnosis, written in school-appropriate language

• 1 follow-up email to clarify next steps for school submission
